Dear library patron,
Every three years, E. H. Butler Library asks all members of the campus – students, faculty, and campus staff – to participate in LibQUAL+®, a library satisfaction survey that examines users’ satisfaction with our library services. Although participation is voluntary, we hope you will complete this survey in its entirety so that your voice will be carried through in the library’s strategic planning efforts and service initiatives. Information gleaned from this survey will be shared with the library and the college as a whole via e-mail, committees, reports, presentations, etc., and your research results, minus any identifying information, may be used in future research by Butler Library to compare data from other years to examine similar topics. Aggregated information and data may be shared, too, through scholarly publications and venues, such as journal articles, conferences, etc.
You must be at least 18 years of age to participate in this research study. Like most survey research, the level of risk to you for taking this survey is minimal, and you may stop the survey at any time, even if incomplete. Besides questions of library satisfaction, the survey asks you to provide some demographic information as well, and optionally you may include your e-mail address so you are eligible for one of the thirty prizes valued at about $15 each. This information is collected solely for the purpose of contacting you in the event of winning a prize, and your e-mail address will not be shared with anyone else. (However, not providing your e-mail address makes you ineligible for receiving a prize as it is the only contact information collected in the survey.)
The LibQUAL+® approach to confidentiality is guided by the ethical standards of the American Psychological Association (see http://www.apa.org/ethics/code/index.aspx). Through the use of sophisticated web technologies, the LibQUAL+® team and E.H. Butler Library take measures to ensure confidentiality of your answers while protecting your online identity. When your survey data is sent to the main database, your answers are separated from your e-mail address before it is stored, to ensure confidentiality. Thus, the likelihood of tracing your survey back to you is very remote.
